2/27/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her day. She verbalized to QMHP-C that her day was going well. QMHP-C informed Ashiyah that this session was going to focus on anger management. 2. Behavior: Ashiyah discussed throughout the session at times on how she was agitated about returning back to CCP. 1.Intervention: QMHP-C began the session by inquiring about Ashiyah’s current thoughts, feelings and emotions. QMHP-C provided Ashiyah with a therapeutic activity via mobile device on anger management to educate Ashiyah on managing her anger by expressing her feelings and thoughts as well as learning not to be reactive when frustrated and upset. QMHP-C employed active listening skills to allow Ashiyah to express her feelings about what makes her upset and frustrated. 2. Intervention: QMHP-C assisted Ashiyah with understanding how her behaviors are projected onto others and how others feel when she's using profanity, escalated tone levels, and defy rules and regulations. At the end of the session, QMHP-C engaged Ashiyah in role-playing to assist Ashiyah with identifying situations, things, people that triggers her anger and frustrations. 1. Response: Ashiyah appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Ashiyah continues to struggle with controlling her anger and frustrations as she reports that when someone yells and screams at her sets her off. She reported that past instances prove that she acts out before thinking of the consequences for her actions. Response: She shared that when her needs go unmet, she also acts out inappropriately with others to include her mother. Through role-playing Ashiyah recognized that she needs consistent support with controlling her emotions and feelings and that through counseling sessions, she is a work in progress.
